Important: Please read the entire manual before installing the Quick Release TM Arch Bar Kit.    (All parts need to be sterilized “autoclavable”).

 

General Warnings, Precautions, and Contraindications: Persons unable to remove the Release Bars due to a physical limitation and/or persons without adequate assistance in the removal of Release Bars  - do not indicate The Quick Release TM Arch Bar Kit for use.

 

Indications for Use:  Casey Surgical Quick Release TM Arch Bar Kit is indicated for use in intermaxillary fixation.

 

Sterilization: Kit contents are sold non-sterile and may be autoclaved or sterile gassed.

 

Description of Device:  The device is composed of four different components, [Adjustable Loop arch bar –two flexible Release Bars (rip cords) – two Release Bar End Gaskets - Erich arch bar] which are installed with ligature wire.  The installed device provides temporary jaw immobilization.  The device incorporates an emergency release mechanism as a safety feature.

 

Installation Instructions:

Size and pre-form the arch bars to fit dental arches. As customary alter smooth arch bar ends after sizing.  [Loop segments on Adjustable Loop arch bar are adjustable to aid in a most uniform channel for placement of flexible Release Bars.]   

 

For improved patient access to the Release Bar handles, it is highly recommended to affix the Adjustable Loop Arch Bar to the maxilla

 

Size Release Bars by inserting each into the Adjustable Loop arch bar channels from desired anterior location and thread to most posterior end of Adjustable Loop arch bar on respective right and left sides. Cut inserted Release Bars off, two or three millimeters, past last posterior loop segment. Remove Release Bars from arch bar channel and alter ends smooth.

 

(RELEASE BAR IS FLEXIBLE FOR EASE OF INSERTION THROUGH LOOP CHANNEL.)

 

 

Note: If loop channel has a hard bend (i.e. going around a cuspid) lightly bending flexible Release Bars, to the shape of formed arch bar, aids in insertion of Release Bar at initial and final placement.

 

If bending of Release Bar is needed, keep the handle of Release Bar in the downward position to the relationship of arch bar so Release Bar handles remain fairly flush to surface of tooth following completion of all wiring.

 

Make note of the positioning of each Release Bar in relation to the arch bar, prior to removal from loop channel, to insure proper replacement of Release Bars.

 

Place Release Bar End Gaskets within most posterior loop segments. Insert Gasket tail into mesial portion of end loop and pull tail through loop until it is fully engaged within loop segment. (FIGURE 2)

 

 

 

 

Wire arch bars to the dental arches the adjustable loop segments, on the Adjustable Loop Arch Bar, are to be projected downward, toward occlusal plane, (scallop edge facing downward toward occlusal plane), when wired to the maxilla. (FIGURE 3)

 

 

If wired to the mandible, the adjustable loops will project upward, toward occlusal plane,

 (Scallop edge facing upward toward occlusal plane).

 

Note: While wiring Adjustable Loop arch bar to dental arch, keep all ligature wire twists (wire rosettes) opposite the occlusal table and away from buccal or facial portion of Adjustable Loop arch bar to eliminate any possible interference to the release mechanism.

 (Keep wire rosettes away from the arch bar to opposing arch bar ligature wiring).

 

After wiring arch bars to dental arch replace Release Bars within Adjustable Loop arch bar ‘loop channel’ and into End Gasket.

 

To bring into intermaxillary fixation, ligature wires are first supported on Adjustable Loop arch bar loop segment and passed between Release Bar (rip cord) and Adjustable Loop Arch Bar buccal surface on both sides of loop segment. Boxing can be employed as in figure 4.

 

Wire to opposing Erich arch bar tab and tighten the ligature wire.  Keep all ligature wire twists (wire rosettes) toward Erich arch bar and away from flexible Release Bar. After ligature wires are tightened, (figure 4 - ARROW) pull ligature wires supported on Adjustable Loop arch bar loop segment forward or toward occlusal plane, away from any wire rosettes, to ensure arch bar to opposing arch bar ligature wiring is tight. 

 

After wiring is complete, grasp handle of flexible Release Bar and pull forward slightly and then back into position to ensure Release Bar is functional.  If too much resistance is present, reduce tension on ligatures until Release Bar is able to move.

 (Do not worry about over tightening ligatures prior to checking Release Bar mobility --- you can get these ligature wires extremely tight and the release will function, yet do check for slight mobility).

 

In case of an emergency, the patient should be instructed to grasp the Release Bars by the handles and firmly pull from mouth releasing their fixation.

 

 

Important Patient Instruction: If the patient removes the Release Bars, thus activating the release mechanism feature, the ligature wires holding the arches together will be released into the oral cavity and will need to be immediately removed by patient.  

 

The patient will need to return to the doctor for rewiring of the Release Bars.

 

Light elastic traction - Bring elastic band into a loop segment “aperture” on Adjustable Loop arch bar and around the opposing Erich arch bar tab to employ light elastic traction.

 

Note: Loop segments on Adjustable Loop arch bar may need to be lightly manipulated open for elastic band placement. (Place a woodson instrument into loop segment aperture and torque loop slightly open to allow the band placement within loop segment).

 

Warning: Utilizing elastic bands arch bar to arch bar discount the safety release feature.

 

Note: Patient is to be instructed not to manipulate or bend flexible Release Bars during appliance use, as excessive manipulation could damage Release Bars.
